 Case Manager

Location: Access Center

Responsibilities:
Under the supervision of the Program Manager, the Case Manager is responsible:

• Provide crisis intervention, case management and follow-up services to a caseload of 25
homeless individuals
• Provide information, referral services, and advocacy to support clients in achieving their
goals
• Develop effective, trusting relationships with clients using a client centered approach that
includes motivational interviewing, harm reduction, and housing first
• Perform a comprehensive needs assessment (screening and intake) for all case managed clients,
and develop a plan that is based on client’s needs, issues and goals
• Maintain accurate documentation of client information (intake, referrals, progress notes and
service delivery)
• Maintain accurate client records and statistical data (manually and via computerized database
system)
• Educate community members (and potential clients) about Access Center services and the issues
faced by the population served
• Participate in on-going self-evaluation, program evaluation and development/modification
• Participate in all case file, staff meetings, trainings and retreats
• Communicate effectively and in a timely manner with management team, peers, collaborators, and
clients
• Report to work as scheduled
• Other duties as assigned

Qualifications/Requirements:

• B.A. or equivalent experience
• Minimum of one (1) year experience in case management
• Valid Driver’s License
• Demonstrated knowledge of issues faced by population served
• Knowledge of social service agencies, especially on the Westside, is a plus
• Experience working in a community-based setting and as part of a team
• Committed to improving the community through empowerment and dignity
• Skilled in non-violent crisis intervention
• Ability to self-motivate and be flexible in a fast paced environment
• Computer literate; able to use computerized database system for client file management
